The top portion of this page is the backside of the Payment Slip. If you'd like to change how you pay for your bill or change
your billing address, please complete this form and mail to Union Wireless. If you have chosen to pay via credit card, you must
fill in your Credit Card Payment information and mail to Union Wireless.
Total Amount Due
Here you'll see the total amount due. Please fill in how much you have included in your check,

AutoPay you will see the total amount that will be withdrawn from your account or credit card,
and you will not need to mail this form to us.
Your first bill reflects charges for a partial month of service (charges from the day you sign up to the end of your billing cycle)
and a full month of service, resulting in a higher than normal bill. After your first bill it should go back to normal.
Payment for your bill should be received on this date. If you have signed up for AutoPay, this is the date funds will be automatically transferred to Union Wireless.
This is the date we issued your bill. Your invoice date should be around the same date each month.
We've allocated an account number for you. You'll need to keep this number handy
whenever you have any questions about your account.
This Month's Charges
Your total charges for each category of plans on your account will be listed here. A break down of these category charges
can be found on Page 2 of your bill.
Last Month's Charges
This section details the previous month's balance and payments received. If there is an outstanding balance before the new charges, this will be listed.
